ECG setup button is not appearing in health>Heart>Electrocardiograms(ECG), screenshot is attached.

Where did you buy your Apple Watch? If you bought it in your country, changing the settings for the region will not enable features that are unavailable for this region, watchOS is country specific.
The ECG is not available for your country, for example:
Sorry, but since the Apple Watch will always download and install the country specific software, this feature can only be used, if Apple gets approval by local authorities, to include it for this region/country.
Until then, you can not use it.
