The current stable release of Sequoia including bug fixes, security updates is macOS 15.3.1 - I would start there.

Mine aren't disappearing, but the "Accessories" widget for Home randomly either works or doesn't, apparently depending on what "mood" it's in. I can click on its edge to get to the Home app, but it's completely out of sync with the Home app. Widget will show a device is on; clicking on that will change it to off on the widget but actually do nothing. Same device in Home app works fine, so it isn't the hub or the network. And then whether the device is actually on or off, the widget will (arbitrarily?) revert back to its prior state, regardless of the actual current state of the device.

Thought I'd try to "fix" it by deleting and re-adding the "Accessories" widget, but now when I go to "Add a new item", nothing happens; clicking on the selected "Home" shows "Loading...", but nothing ever loads, so now I can't even get back to having it work only some of the time.

It's a bit of a failed design/implementation for what appears to be a distributed app. Seems there should be some kind of macOS "service" that connects to the "hub(s)" (in this case, a HomePod mini model MY5H2LL/A) to keep track of the device states, and both the "Home" app AND the corresponding "Accessories" widget should communicate with the service (basically just having the app and widget act as UIs for the service).

Apple app/widget, communicating with an Apple device, running on an Apple OS, on Apple hardware. Responsible party: Apple.
