iPhone's device name keeps changing, and safety check shows multiple devices

Dif occasions I have logged in to Appleid.com check devices…..go to iCloud and sign out of all devices. Various apps that keep account of how you log in have shown multiple devices of this same model but some will just say iPhone some will say names my device has had along with the top device stating “this device” followed by the rest. My personal share contact poster turns off as well changes the actual poster I made. My safety check always says either it can’t stop sharing with Home go to app and stop share or Fitness and sometimes both and I don’t even have them installed or enabled that msg is in emergency reset then in the safety check the first check with apps sharing or signed in it will say I’m not sharing any information with any apps or 3rd party apps half of my permissions are greyed out I have not been able to change since like 1-2 weeks after I got the phone and there’s a lot more…. Like actual photo, video texts files changes additions and altered time, date, format and blocked from being able to open, view etc etc
