See Recover your iTunes library from your iPod or iOS device - Apple Community. The easiest method may be a temporary subscription to the Apple Music service (assuming that works with iOS 12), merging the music on the device into an iCloud Music Library which you then download to a computer and backup for security.
Alternatively I've had success using raw file system access with iFunBox to copy off the files, and the two scripts ImportDevicePlaylists and SyncStats to rebuild playlists and stats. See Recover media from an iPod - Apple Community for more.

I would not suggest Apple Music for that, as Apple Music will generate DRM protected files that will stop working once the subscription ends.
For that, iTunes Match is a better option, since it provides non DRM protected tracks that can be kept past the subscription.
Beyond that, extracting them from the iPod would be my first choice.

Hi Phil,
That was certainly the case in the early days of Apple Music, but Apple have long since folded in the DRM free match/upload feature from iTunes Match so that cancelling either service, with your own content downloaded, doesn't leave you in DRM h*ll. (Such sensitive ears here!) For this reason nobody should pay for both services at the same time.
Apple Music can also match/upload tracks on an iOS device, whereas iTunes Match (as far as I understand it) only implements that function on a computer.

Definitely should not pay for both.
Was not aware Apple Music had stopped DRM protecting matched / redownloaded files. Good to know.
Though now I wonder if the matching and upload will work from an iPod on iOs 12.

Yes, iOS 12 is quite old now. I cannot say for certain that method would work. Not really something I can test.

There's no native way to do with Apple apps. There are apps which say they can do it but I've never used them so can't vouch for any of them.
