I’ve tried disabling it in every app, but unless I’m typing in a silent room, every word it hears is transcribed like it’s taking dictation. It’s impossible to type a coherent sentence, rendering this iPad nearly useless.

You appear to have the Dictation feature enabled. From iPad settings:
Settings > General > Keyboard > [Dictation] Enable Dictation - set to OFF
