Why is my iPhone alarm deactivating itself and how can I prevent it?

It’s been two days now and my alarm has been deactivating itself … yesterday I asked Siri to set my alarm for 4.00 am and when I jumped up it was past 4 and I panicked checked my phone as to why my alarm didn’t ring I see it had deactivated itself. Last night I set my alarm twice, once for 4 am and a second at 4.30, I jump up luckily at 4.13 to see it hadn’t gone off again!?! Why is that so and yes my attention awareness features setting is switched OFF so why is this so? Is apple not reliable anymore to get me up early for work? This is a very serious problem can we get a solution to this please?? I am very disappoint and upset and I need an explanation and how it will not happen again!!

Apple is not liable for you not getting up.

Update your Computer:

The first thing that comes to mind is to update your iPhone. So, what's the model? You can update on the iPhone itself, through use of a computer, or through Cellular (if an iPhone 12 with 5G or later).

Use my User Tips:

IMPORTANT: If you have an option to update, however, then be certain you backup your iPhone first. That way, you will have something to restore your iPhone from should anything go wrong with the update

That’s the thing I had asked Siri to set my alarm which was the first time it didn’t go off … the next night I set two alarms which still didn’t go off. I had to restart my phone but it’s just weird and frustrating how that can happen.

Investigating this:

When did this begin? I did this just start happening out of nowhere? Or was there something you had done prior to this all occurring? It may be a bug in an update.

"Ok … so if Apple isn’t liable - I get that. but if I am using the IOS 18.3.2 version on my iPhone 15 pro max and I set two alarms which none of them go off, then whose problem is it??"
