I have an Apple Watch series 10 cellular family connected and I can’t download the vitals appl my watch is up to date. If I go on my watch and try to get it, it says requires iPhone and on iPhone it says watch Os 11 + only

That's correct. you can't get that on a watch set up as family connected. It requires a watch that is paired with an iPhone.
Then what they are talking about is not a family setup phone. Either they have their own iPhones and are part of a family sharing group, or someone has paired the watches with an iPhone and not actually used family setup.
Not sure about that. I have friends with the newest SE and cellular as well as family setup and they still have the vitals app and the sleep app.
