I’m on iOS 18.3.2 and I can add world clock widget to lock screen but can’t change the default three cities to anything else. Update the cities in world clock app itself doesn’t work.

Are you trying to change the city to a different city in the same time zone you are in? Not all cities in the world are available options in the world clock. You typically just choose the closest city to your location, which is in your time zone. For example, I live Florida, but for the World Clock, Washington D.C., is the closest city and is in my correct time zone. If this isn't what you are asking about, please provide more details.
I’m trying to change the cities in the world clock widget in the LOCK SCREEN. When I press the widget in lock screen the world clock app pops up. However, it’s always CUP, TOK, and SYD no matter what I chose in World Clock app.

I tried that and it works for me. Try the same steps as I stated above.
When you are editing your Home Screen, if you tap on the clock widget, you can set it to about any place in the world. I made a gif below for you.
