I have been successfully using location based naming on my IPhone 16 ProMax. However when I look at the list of recordings I see that since April 27 it is not naming that way anymore. No amount of toggling and rebooting is working. I am running the latest iOS 18.5

Is your location still shared with VoiceMemos? If location-based naming is on and not working, Voice Memos might not have your location to begin with.
Try going to settings > privacy and security > location services > Voice Memos. If it's set to never, change it to "While Using the App" and try location-based naming again.
