I've installed IOS 18.5 on the iPhone 13 but Password & Security doesn't appear in the Settings app and the Password app shows no provision for two-factor authentication.

You need to go to Settings, then tap your name on top and you should see the option for Password & Security (or possibly Sign-In & Security). More information including that location is available here:

Not sure what region you are in, but if you do not see the option there, you might want to check if it is available in your region.

Yes. Two Factor Authentication is added to your account not your device, so you must be signed into an Apple Account.
See this regarding 2FA. Two-factor authentication for Apple Account - Apple Support There is no setting named as you indicate. There is Privacy & Security and Face ID & Passcode, depending on what exactly you are looking for.
