Six hours later, after countless tests and useless suggestions from all over the internet, I found where the problem was. Apple menu => System Settings => Network => Firewall 'Options' button => ContinuityCaptureAgent set to 'Block incoming connections.' Changed this to 'Allow incoming connections' and Continuity Camera now works.

Turn Off Firewall and Reboot:

Are you using a firewall on your Mac? If so, turn it off, and then restart both devices. Report back if that fixes this.
