upgraded to 16 Pro Max today - STILL have this issue.

Apple is Apple. These are just user-to-user support forums. If you think Apple needs to learn of this(and, yes, they do), then report it to them.
So, do your Part: Provide Apple with feedback on this, letting them know about this issue. Apple may not get back to you directly, but the more feedback they receive on this, the more they will know what is occurring, and what bug fixes to include in updates to come. Apple will know nothing of this, if you don't report it to them.

Do you wear Apple Watch by chance? If you do, when your iPhone is locked, you won't notifications on your phone. If your phone is unlocked, you will get notifications on your phone. This is exactly how Apple designed it to work. Again, if you wear Apple Watch.
