How does the iPhone Upgrade Program work, where I pay monthly and get the newest iPhone model every year, and where can I apply for this program?

It works exactly like that.
When you purchase an iPhone, if you choose the iPhone Upgrade program during the purchase process as your method of payment, you will be enrolled in the program. To upgrade you need to complete at least 12 months worth of payments.
