This used to happen automatically but somewhere I assume an update changed settings. Everything I’ve found online has steps that do not exist in iOS 18 settings.

What exactly do you mean by "App sync"?
When setting up a new device from the backup of a new one or when using Quick Start, apps installed on the first device will be set to download to the other device. This is a one time thing, only during setup of a new device.

Do you perhaps mean having Apps that are downloaded to one device automatically download to the other?
That option exists in Settings ➜ Apps ➜ App Store ➜ Automatic Downloads ➜ App Downloads.
Both devices need to be signed to the same Apple ID for iCloud and Media & Purchases.
