Unable to remove puzzle icon from search bar on my iPad OS 26

I don’t have any extensions installed so why does my Safari Private browser show the puzzle piece.

There is no way to remove the puzzle icon.

When the puzzle icon is pressed, it takes me to the App Store extensions section. This hardly seems private having a permanent icon with link watching over you?

Strangely, my iPhone’s iOS26 Safari browser does not have this puzzle icon showing in Safari - only on my iPad - which leads me question whether this is an intended behaviour on the iPad, or a bug?

The reason for its appearance is due to an app on your iPad capable of enabling a Safari Extension. You have to determine which app you installed, and delete it.

Only you can or should know which apps you installed on your iPad, which brings us to:

All I can assume is some extension keeps installing itself on my iPad, as I have already performed 2 factory resets on the device, yet nothing changes with the iPad’s Safari’s behaviour when rebooted from a fresh install?

Nothing keeps installing itself on an iPad without your knowledge or consent, unless of course that iPad is owned, controlled, or otherwise managed by someone other than you. However, you would have had to grant your consent for that to occur in the first place. Since it appears you have no knowledge of that ever having been the case, and taking into account the fact you have had past experience with some apparent malicious intent, then I can only suggest you contact Apple directly to physically examine that iPad, reset it to factory defaults, and proceed from there. I understand your concerns but these things don't happen by chance.

If I had to guess, which I do, based upon nothing other than what you wrote, then someone — probably someone you know — has been messing with your Apple devices.

By all means contact Apple and have them put your fears to rest. Use the Support link below, and please let us know what they have to say.

Thanks for your suggestion John, but unfortunately your above instructions do not resolve the problem (removing the puzzle icon). More concerningly, under Safari App settings, it tells me that I don’t have any extensions installed.

Could it be that the iPad device is infected with malware?

Or alternatively, perhaps I have a modified version of iPad OS 26 that keeps installing a hidden extension somewhere?

I can’t understand how this puzzle icon is permanently active in my browser search bar, especially when the system tells me that I do NOT have any extensions installed?

My iPhone iOS 26 doesn’t have this issue.

Sadly I am left doubting whether my device is running tampered software, as nobody has confirmed the Puzzle icons in the Safari search bar is expected behaviour on iPad OS 26 & 26.1 when no sections are installed?

iOS 26 & 26.1 do not have this problem 🤔

I am confident there are no visible extensions running from my system - so that makes me question whether there is a hidden extension somewhere. I know that I have not installed any extensions - and my system tells me that too - so I am stumped & unable to understand why the puzzle icon remains. 🤔

Sadly, my Apple devices have been compromised previously over the years - so this puzzle icon problem is extremely frustrating - when a user is unable to be sure that privacy compromising malware is NOT running in the background, like I a concealed extension somewhere 😱
