I just upgraded to IOS 26.0.1 and everything's fine, unless I inadvertently tap this icon, which won't go away...
If I leave it alone everything works, but if I tap it then no apps will open, only a small rectangular black box near the top of the screen telling me which app I've tapped, but the app won't open. I want rid of this icon as it's a liability.

It's the Hover Text icon.
You can disable in Settings ➜ Accessibility ➜ Hover Text.
It should show a large label at the top with the text that's under the pointer or finger.
I'm guessing you activated it at some point and forgot about it.
