To fix missing video thumbnails in Safari:

Turn off Prevent Cross-Site Tracking
Disable all extensions/content blockers
Allow Auto-Play
Clear website data
Turn off Reader mode / Zoom
On iPhone, disable Low Power Mode and Reduce Motion

These resolve nearly all cases.
