So the phone is on the nightstand without being touched, yet it seems to eagerly go into "silent mode" after a while.

Phone doesn't need to be touched, if you look at your screen it will minimize the volume.

Check face id settings and turn off attention aware features and then do a test alarm.

There are times when I forget to turn off my alarm jump in the shower and the alarm just keeps ringing until I either look at the phone or turn it off.

Yes it happens if you have face attention awareness enable, it lowers all notification sound when you look at your phone.
