How do I regain access to my Apple Account when unable to change security questions?

Someone is trying to steal my Apple account. I tried changing my password and security questions, but Apple won't let me change or enter the answers to the questions. How do I get my account back?

If you didn’t have two-factor authentication enabled, it is possible your account has been compromised, and is no longer yours.

If you did (do) have two-factor authentication enabled, you don’t have security questions.

If you don’t have two-factor authentication enabled, enable it. Or try to.
