In my setup using the M4 Mac Mini with macOS 26.3 - refer below

When I adjust the Sound Level for the Internal Speaker

Each time I adjust Up or Down, like in your image

The image will appear briefly ( flash ON ) and then Flash OFF

When I adjust the Sound Level again, it will appear briefly ( flash ON ) and then Flash OFF

This are normal behaviours

Now, you mentioned " I tried the NVRAM " which seems to imply you tried Resetting the NVRAM ?

The proper way to reset the NVRAM on Apple Silicon computers is to shutdown the computer for about 30 seconds

Then Start the computer in a normal way

Is this what you tried or some other way ?

If some other way, how exactly what that performed ?

Do you mean the volume appears momentarily when you use the Fn keys to increase or decrease the volume like this:

If that's what you're referring to it's normal when you use the Fn keys to change the volume.
