I have Lupus and unfortunately have had memory impairment due to attacks on the brain. Therefore, I cannot remember details of my phone conversations. This affects my ability to do my job. When I bought my iphone4 yesterday I wrongly assumed there would be an "app" to record my personal calls. I have no problem letting my clients know Im needing to record due to my disability.
My solution, although not solved by an integrated app is a working solution and may be applicable for others. I have glued a small square of velcro (1 inch) onto the back of my "ottobox" phone protector. I purchased a 4 inch digital recorder which uses the new small digital flash media. When I'm not wearing the iphone, I attach my recorder to the velcro patch. I attach one end of a "y" cable into the 3.5mm headphone jack of the iphone and plug the other end of the "y" cable to my digital recorder. Its easy to press "record" when I need to record a call, Incoming or Outgoing. makes no matter. So, until apple is able to feel good about creating an internal software solution, this is a solution for those who may also need a digital record of their phone conversations. It also enables u to later use an application on your laptop and have the digital file easily translated into a written file using a speech to text software application should you have a need for a hard copy too. I use this and it makes it easy for me to check off each detail I need to accomplish. Hey, we who have "impairments" have to get creative! Hope this helps someone.
