After revisiting this for about the 5th time and trying the many different methods in this thread, the only way I've been able to restore my Internal Speakers was to plug in a 3.5mm headphone jack and leave it plugged in.
This causes the OS to register "Headphones" as the connected output device in Sound (System Preferences). From here, you can then set the "Use audio port for" setting to "Sound Input." This causes the audio port to switch to input mode which in turn brings back the Internal Speakers as the default Output device.
Essentially what you're left with is a dual-purpose stereo jack for your audio port: fix for your Internal Speakers and removable dust cover.
exact same problem here! mine is surely a problem of software. I tried doing the PRAM trick and I got the 'bong'' sound after my MBP restarted. But when it proceeded to the login page, the sound is disabled again...
I too had the same problem. I think I know what casued mine to start having trouble. I had my iPhone earbuds in my gym backpack, next to gum. I found a wrapped piece of gum stuck onto the earbud plug. I think I noticed it too late and had already inserted it into my MacBook Pro. Once I noticed it, I removed the gum. It was from there that I noticed the built-in internal speaker choice was gone, or came, and then went. I used a cotton swab per earlier postings and it worked.
I think that dust and/or, in my case, gum, may contribute to the problem. The internal speaker choice has reappeared and all is well.

I wanted to thank sanjampet for great answer that worked! I tried rebuilding my plist file, then used the end of a wooden match stick to poke into the audio output port a bit, but neither of those worked. Using the utilities located AudioMIDI app. worked like a charm to get the darn system preference program to realize internal speakers existed. Something wrong with the system preferences program I guess 'cause you can see the internal speakers definitely exist when you go to "About this Mac" to see the hardware configuration. Thanks!
Just hit the same issue myself today. New 27" iMac. Never used earphones or external speakers with the system whatsoever. Followed every trick given so far in every thread about this issue, and none have worked. Did not do the toothpick idea (no one should ever...EVER...shove anything into a computer, unless its out of warranty.)
Start up chime is active, so its not a hardware issue per se. Definetely software.
I have a 3pm call booked with Apple for tomorrow, so hoping they have a resolution to this matter. Otherwise, back in the box and back to the store to get a replacement.
awesome! Too nervous to poke around with a toothpick, so tried wiping the .plist files - no luck.
Looked around for a toothpick, couldn't find one, so tried, of all things, a little plastic LEGO shovel - worked like a charm
THANKS
Same problem happenes to me with the block sign on the volume control.
I realise it happened after I used the headphones and external speakers.
After trying the many suggestions from the forum, I left with poking it LOL
I didn't bother to use the toothpick as I have my earphones with me.
Poke with the earphone jack and true enough the internal speakers selection
is back!
Advice: Go easy the next time you insert any jacks into Macs.
Hey,
I just had this problem and found a VERY simple solution posted on youtube. Basically, do the following:
1.) plug in a set of headphones
2.) go to the sound menu in system preferences
3.) make sure "Headphones" is displayed in the outputs list
4.) return to the system preferences menu
5.) unplug headphones
6.) return to the "Sound" menu and internal speakers should be listed and functioning
If that didn't make too much sense, here is a link to the youtube video I found:

Hope that helps
JRyankersh, thanks a bunch for your solution. Worked for me. In my case, I think the "voila sound device" did something to my speakers (the speaker device was just gone like others explained above).
You trick worked, thanks!

Tried 1, 2 then 3 - no luck
Couldn't be bothered to shut down to try 4 or 5
Repeated step 3 multiple times - in out, in out
The red light was visible in the port, would go out momentarily then come back on
Finally after quickly doing this about 4 times the red light went out, stayed out and my internal speakers are back
😀
Have had this a few times, usually after travelling / a lot of rough headphone use (...by that i mean accidentally tugging on inserted headphones while on a plane or something). The only thing that ever works is the "insert headphones and then pull out very slowly" thing... takes several tries.
Had the same issue. Plugged a new hedphone and it worked just fine. The next day the sound icon looked disabled, as the built-in speaker itself. Tried 2 and 3 and didn't worked for me. But I've a found this video that helped me: https://www.youtube.com/watch?v=u58JlKrTRqE I believe it the same as number 1 tip. =) Now speakers work just fine.
I tried the toothpick trick but nothing happened so I plugged in my earphones to the jack and hey presto - the hole must just collect dust and stuff so a little wiggle sorted out the problem - thanks!
Just got this problem, after using my Apple EarPods with Remote and Mic ( they came with my iPhone 6S). The red digigal light was spotted inside the earphones hole. I put my old Apple EarBuds and it runs again.
I saw this post and just inserted the headphones, which added a "headphones" output option. I played a sound effect from the Sound preferences to make sure the sound was working through headphones then when I removed the headphones the "Internal Speakers" option came back. Thanks!
it's super helpful. i have tried reinstall the system and unplug out every wire connected to internal speaker. but using a toothpick or a chopstick is totally helpful, which i really don't know why. any way thank you so much.!!!!
