How do I create blank file folder labels document using Avery template 5366?

I want to make file folder labels using Avery template #5366. How do I do this in Pages? At Christmas I created address labels and now when I try to create new labels, it just keeps sending me back to the address labels page and I can't figure out how to make a new document. I really do not want to delete the old document. Any help would be appreciated.

I am having the same challenge finding a way to make File Folder labels to then use on my Macbk Air (10.8.4 version). The above solution did not work as well for me...still having to make too many individual adjustments for spacing concens.

From the Avery Page, I downloaded the "Template only (works with PC and Mac)" version. I then tried opening the document in Pages (an image showed up but did not work) and Open office (got there but still way too many adjustments needed).

Any ideas?
