Cannot create new image even though verify disk says it appears to be ok
i cant create a new image using disc utility (input output error) even though verify disk says its ok. what is wrong?

Where, what is the source & destination exactly?

Is it a commercial DVD or one you burnt yourself?
i tested it on both types. some of the ones i burnt myself i can create a new image. some i cannot. the same goes for some "commercial" dvds i tried.
Well, input/output errors, if not due to corruption, then can be from low power, (Drive or Optical dipping off line), bad Filenames or folder names, (which may be caches or even flaky RAM.
If you have Console in Applications Utilities open when try a failing one, do we get any better clues?
it says preparing imaging engine. then reading whole disk. then after almost an hour it says unable to create "name of disk.dmg" input/output error.
Do you have 3-4 times the size of this DVD as free space on the Hard Drive?

I'd give a try with the free demo of Dragon Burn & see if we get the same problem...
i tried with data dvd option, but is says to drag files and folders into the list to burn. i'm trying to copy the dvd, and dragging files and folders will not get me the entire dvd because the sum of the contents that i can select are less than the total space occupied on the dvd. is there any other reasons why disc utility keeps giving me an input output error on certain discs but not others besides the reasons you already listed?
it's stuck at 86%. i also can't cancel out and had to force quit. if there's something wrong with the dvd, why would the "verify disk" function return an "appears to be ok" message? if it's a double layered dvd, would that make a difference? would changing the default data format make a difference? i also found this in a different log: sam multimedia: read or write failed, sense_key = 0x03, asc = 0x02, ascq = 0x00
Verify Disk only checks the Structure & where files/folders start & end, but if there's a bad Sector DU won't find that unless it's in the Directory, it doesn't check the Data integrity.
Say you're at 7th & Main, you want to get to 1st & Main, good directions tell you to go west 6 blocks... but you find the street at 3rd & Main is torn up.
I think the DVD is damaged, but can you find another DVD-DL that does 100%?
if the definition of dual layered is that the maximum capacity is beyond 4.7 gb, then i was able to create a new image of a dvd that was over 5 gb in size. the one i'm trying to create an image for is over 7 gb and fails at 86%.
Well, it's not if you can create one, but if you can create one from the DVD, but I'm not sure how you created it.
We need a DVD at least the size of the bad one to insure it's not something else, DVD-DL can hold 8.5GB/7.92GiB.
