A couple of your Fan (Ventilator) speeds seem to be a little off and that is probably what the Apple Hardware Test is reporting.
Generally the default speeds are set to even numbered speeds (1000, 1200, 1300 or 1400 RPM) and this maybe the reason for the 4MOT/4/40000003:HD-1258 if the fan is actually supposed to be at 1300 RPM.
I also see that the CPU fan is at 938 RPM instead of what probably should be 1000 RPM.....
Are you using any third party fan control utilities? If not then you may have a dust, moisture or power problem that is dragging down your HD and CPU fans.
What I would do first.
1. Shutdown and disconnect everything from the iMac.
2. Lay it face down on a soft towel with the stand hanging over the edge of your desk.
3. Next i would inspect and then vigorously vacuum out the air intakes (the bottom grill work and small air intake on the back) to make sure that they a clear and free from any dust accumulation.
4. Stand up, reconnect, startup and re-check your fan speeds again with iStat Pro or iStat Menus.
Now if the Fan speeds are still out of range, then the iMac may need to be internally cleaned and/or serviced.
