Please help me read this hardware error code?

I do hear one of the SATA disks in my machine acting up. Strange behavior lately. I've been through this HD failure scenario before...the

Apple hardware test issue confirms an issue...

Does this error tell me which disk is potentially failing? Which SATA slot?

The new HD in question was reformatted on my Mac Pro. Now however, it won't allow me to install Lion on the disc. When I check it out in Disc Utility it's fine. So I try and reformat and I get the error. However, I took it to another machine and it mounted and reformatted fine. Now I've brought it back to the Mac Pro and it's willing to allow a system install. Should I be concerned about using it as a system disk with the recent disc mount errors?

Please help me read this hardware error code?
