Chronic permissions repair problem?

I've been having to repair my iMac with Disk Utility every day for a while now. I've tried MacKeeper and anything else I can think of. After about a day, it begins to really slow down to a crawl and then seems to have trouble finding the folders (directory issues?). Then I run utilities and I find all this (see below). It's the same every time.

I've done all the Malware checks and I've tried restarting with Command + R to reset the password in the terminal (won't bring up that hidden repair utility at all, just takes me to the user login screen). I know this isn't right, because the first few months or year that I had the machine, I rarely had to run Disk Utilities. Now it's every day. Anyone have a clue what's up?
