Just emailed firstname.lastname@example.org as purdiecomms suggested and got an autoreply straight back - they advised me to reply with my Legal Entity name and the email used on my application, to speed up the reply process.
Also, they sent this link that says you usually have to wait two business days to have your iTunes Connect account approved (wish they said that in iBooks Author as the way it's set up suggests that it all happens immediately):

Posting the same thing in different places does not help.
I have replied already ... read and digest.. first thing is to be certain the information you supplied if full, valid and complete.

I respectfully disagree. I didn't start a new thread, just added my experience above to an existing one, in the hope it might help others (as others' comments in various other threads on the same topic have helped me). There are always multiple threads on every topic, of varying helpfulness, and you never know which ones might get replies...
