I was attempting to update my phone to iOS 5.1.1 when this message popped up:
"There are purchased items on the iPhone that have not been transferred to your iTunes library. You should transfer these items to your iTunes library before updating this iPhone. Are you sure you want to continue?"
iPhone 4, Mac OS X (10.7.2), iCal
Transfer purchases first, otherwise they will be lost. You could always download them again, but not everything may still be available in the Store.
With the phone connected, launch iTunes and select Transfer Purchases from the File menu.

I have just had this same problem of the transfer of purchases being blanked out on my MacBook . I clicked on Preferences - Store- Automatic Downloads and clicked for iTunes to automatically download new purchases made on other devices. It's just crazy that this is not a default setting.
Intuitive functionality used to be the hallmark of the brand. : P It's beyond me how they can let things like this go.
