Unfortunately it appears to me that what happens is that your IPod will tell you that you have songs on the IPod that are not in the Library and would you like to put them in the Library. The way the choice is offered says that if you say "No", the song will be deleted, but it's unclear if it will only be deleted from the IPod or also from ITunes. If someone else has experienced this and knows for sure that the song is only deleted from the IPod, please advise. I said "Yes" to putting the song in my Library and the only songs that were added to the Library were duplicates of songs that i had deleted from the Library...
Had the very same problem until a few months ago. Here is the way to fix it but it involves restoring iPod settings so some follow up needed to get things back to the way you had things set up before.
...now you need to redo settings you already had in place.

If ALL of your songs are currently in your iTunes library, I would just do a Restore, as mentioned above (and elsewhere in this topic). Then re-sync the iPod as desired. There appears to be some data corruption on the iPod, if the same songs keep popping up as NOT being in your iTunes library (when they are). There may also be some other data-related problems on the iPod that you are not seeing.
Select the iPod in the iTunes sidebar. Over to the right, go to the Summary tab. The Restore button is there. This will erase the iPod (and reinstall its software), but if all the content is in your iTunes library, it should be easy to re-sync it.
It's slightly more complicated than that--what i seem to have is some exact duplicates but many of the duplicates are different bit-rate and size versions of the exact same song--i do not know how this happened because i definitely did not download the same song several times but somehow iTunes did...therefore, what i'm trying to do is go through the Library and eliminate the lower bit rate and size versions of the songs first--then i thought the IPod would just sync with the library and eliminate all those low bit rate songs from itself. If Restore accomplishes the same thing i am willing to do that--or is it possible that i should do a one-time "Manual" sync to accomplish this? or will i just get back the same message about my iPod having songs that are not in the Library ?
The most straightforward way is the get your iTunes library the way you want it (with duplicates removed) first, Restore the iPod (to give it a clean start), and sync the iPod from your iTunes library. When using automatic syncing, what's on the iPod reflects your iTunes library (based on how you set up the Music tab for the iPod), so your iTunes library needs to be as you want it to be.
If you use the Manually manage music setting, there is no direct relationship between what is on the iPod and what is in your iTunes library. You can even have something on the iPod that is not in your iTunes library.
When you go from automatic to manual, what is currently on the iPod will remain. You can then remove items manually from the iPod, using the music list under the iPod (in the iTunes sidebar). However, when you change it back from manual to automatic, that sync will be based on how you set up the Music tab for the iPod (not what is currently on the iPod). In other words, songs that are currently on the iPod may be replaced (based on how you set up the Music tab for the iPod). That may resolve the problem. But if data corruption on the iPod is causing this problem, it may not.
That is why I think the best approach is the get your iTunes library the way you want it to be (by removing the duplicates), then Restore the iPod (so that it is completely empty of songs), then re-sync it from your iTunes library.
I purchased a song on iTunes last night on my PC. I dragged it to my iPod touch 4 gen. manually, and now is showing up twice. The first appearance is actually playable and can be deleted by swiping. The second is only half the track name, when "played" it shows the "no album art" logo and then immediately skips to the next track. This can not be swiped to delete. When I dock my iPod to my PC and sync it shows only the one track (the playable one) under the media on my iPod. I've tried resetting the iPod, powering it down and back up...I'd like to use restore as a last resort if at all possible.
If all of your songs on the iPod touch are currently in your iTunes library, you should probably do a Restore.
But if you absolutely do not want to do it, what happens when you delete the "real" instance of the song on the iPod touch? Does the "fake" duplicate (the one that does not play) remain in the iPod touch, or does it also get deleted?
If all of the songs currently on the iPod touch are currently in your iTunes library, you may want to set up automatic syncing in the following way. Create a new playlist in iTunes (in your iTunes library) and put all of the songs you want on the iPod touch (including that new song) on that playlist. On the iPod touch's Music tab (in iTunes), set it up to automaticlly sync that ONE playlist. Apply this change. NOTE: The current music content of the iPod touch will be replaced by the songs on that playlist.
Hopefully, by doing it this way, instead of manually adding one song at a time, you will avoid this duplicate song issue. The other benefit is that you can update the playlist at any time, whether the iPod touch is connected or not. Whenever you update that playlist, any changes will be automatically synced to the iPod touch the next time it is connected (or when you click the Sync button).
When I delete it, the fake instance still is visible. I downloaded two songs last night, both via my PC; and the second song is having no issues...
I restored it a couple of weeks ago, however couldn't add my wallpaper back to my iPod, thus restored to my most recent back up. I just turned off automatic downloads and when I went into iTunes on my iPod and to my purchased music, it showed that the music was already downloaded, although I had just deleted it. Now I can't re-download it. At least not on my iPod...
