Diagnosing cause of kernel panic on a Mac Pro

I've got an early 2009 Mac Pro with the following specs:

2.66 GHz Quad-Core Intel Xeon

16 GB 1066 MHz DDR3 ECC

NVIDIA GeForce GT 120 512 MB

OS X 10.8.1

Problem is, at random times, my Mac would just shut down and then restart after I clicked through the "gray your Mac shut down without telling you because it's selfish" warning screen.

I took it to a local computer shop and they couldn't really tell, and suggested I do a clean install of Mountain Lion. I did that, got everything back on from Time Machine (just Documents and Music, no Libraries per the technician's instructions), and then last night it did it again. Restarted while I was working on a web site. No notice.

I wondered if someone could help me diagnose the problem or problems(s), since I can't make ANY sense of the sytem report.

Here is the complete system report when the panic occurred. Thanks for ANY help at all you can provide.

It's definitely a weird bug. I myself have been using the latest versions of all the extensions, i.e. the ones included in 10.8.2 Supplemental, including IOGraphicsFamily.kext 2.3.5 and IOPCIFamily.kext 2.7.2, and there have been days when I have had two and even three KPs, but on the other hand I have now been using my Mac for 10 straight days without a single KP. This does not appear to be connected to anything in my use of the machine, since I have been doing the same kind of tasks all that time.

(10.8.2 should definitely have updated IOGraphicsFamily.kext to 2.3.5 on your machine. I find it strange that it has not, unless there is something different in your machine configuration that makes the updater refrain from updating it.)

I have another on-going issue (the Finder is quite sluggish, for no apparent reason), but I don't dare restart my computer, lest the KPs start occurring daily again!

Nov 6, 2012 1:59 PM in response to kevloucat

Any of you brave souls tried installing the new Nvidia drivers released on 10.1.12?

This post suggests that they may be experimental.

http://www.tonymacx86.com/257-nvidia-releases-mountain-lion-10-8-2-graphics-driv ers.html

But, they are certainly available on the Nvidia web site.

http://www.nvidia.com/object/macosx-304.00.05f02-driver.html

I have 3 GForce 120s which are supposed to be covered by this upgrade.

Note: There is a second install for CUDA

http://www.nvidia.com/object/cuda-mac-driver.html

I have no idea how to query what my current driver versions are so I'm a little nervous but I'm seriously tired of the panics.

I have a Mac Pro 2008, from Snow Leopard I upgraded to Mountain Lion. I had this type of crashes almost daily. Then I did a clean install of ML, with very little applications. And still those **** crashes were driving me crazy.

I went back to a clean install of Snow Leopard and my computer is rock solid after two months not a single crash.

I think ML is optimized for newer computers. I miss iCloud support but I decided to use gmail anyway.

My two cents.

Nov 10, 2012 6:58 AM in response to MacLeon

Seems 2008 is not just "some models" but every owner has similar story now about ML.

And of course dual boot for those times you do need ML around (keep it handy on a partition or somewhere).

Too bad ML can't be run in an SL VM instead of the other way around.

And this is the kind of thing, not whether there was a real 2012 6,1 model, but is there even real support for a 4 yr old model that was suppose to offer the first (tentative,, experimental?) 64-bit mode support.

Nov 20, 2012 10:21 AM in response to Pierre Igot

Bummer.

It really does seem to have worked for me. 5 more days. No reboots. No KPs. I'm a developer / designer and I stress the heck out of my machine. It's now 14 days without a KP. Here was my last KP to make sure we are getting the same alert.

I have 3 GeForce 120 cards on 3 LED Cinema Displays on an Early 2009 Mac Pro

Nov 25, 2012 6:34 PM in response to cryptomagic

Still going OK for you? I am going nuts here. After a 5 day reprieve (for whatever reason), I have had 2, 3 or 4 KPs per day for the past few days, not doing anything different. I've just tried to reinstall the NVIDIA driver (1.3.4.0 [304.00.05f02]) and have also installed the CUDA driver, but it has not helped.

I'm getting sick of this and have ordered a dual-link DVI to MiniDisplayPort adapter, so that I can connect both my 30" screens to the same GPU and remove the other one if necessary. It's expensive, but I can't go on with several KPs per day like this. I am really disappointed in Apple's treatment of this issue. I can't believe it's been several months now without any sign of a fix.

If things are OK for you, I'd really be curious to know what's different between your system and mine. Mine is an early 2009 Mac Pro with TWO GeForce 120 cards driving two 30" displays.

I have the following system extensions in System/Library/Extensions/:

CUDA.kext [no version info]

GeForce.kext 8.1.0

GeForceGA.plugin 8.1.0 (304.00.05f02)

GeForceGLDriver.bundle 8.1.0 (304.00.05f02)

GeForceVADriver.bundle 8.1.0 (304.00.05f02)

IOGraphicsFamily.kext 2.3.5

IONDRVSupport.kext 2.3.5

IOPCIFamily.kext 2.7.2

NVDAGF100Hal.kext 1.3.4.0 (304.00.05f02)

NVDAGK100Hal.kext 1.3.4.0 (304.00.05f02)

NVDAResman.kext 1.3.4.0 (304.00.05f02)

System.kext 12.2.0

Anything different from what you have?

I get the exact same alert in each KP:

That panic is a machine-check. Those are Hardware Problems.

You have no non-Apple kernel extensions added, so this is an all-Apple problem.

It is conceivable that a memory problem caused this, but I think you may have a bad processor or bad cache.

Was that an Apple technician? Did your technician run diagnostics while he was there? If not, you should.

This could be a case for the basketball rule (it is the fault of the guy who touched it last). Check to be sure those new RAMs are correctly seated.

There was another CUDA update from nVidia so I applied that and I've been OK for 5 days. I'm starting to wonder if that is actually fixing anything.

I'll try to check this thread more often. How do I generate that list of extensions you provided?

I'm a programmer and this is my primary machine so I'm motivated to get this fixed. Have you posted it on any nVidia forums?

BSD process name corresponding to current thread:

Mac Pro, Earl 2009, 2x NVIDIA GeForce GT 120 512 MB, OSX 10.8.2, three monitors.

However mine is only happening every few weeks, which makes it rather hard to track down. I'm suspecting it something to do with VRAM - some kind of video driver problem being revealed at random by particular usage patterns. I usually have lots of windows open, mostly Chrome.
