Diagnosing cause of kernel panic on a Mac Pro

I've got an early 2009 Mac Pro with the following specs:

2.66 GHz Quad-Core Intel Xeon

16 GB 1066 MHz DDR3 ECC

NVIDIA GeForce GT 120 512 MB

OS X 10.8.1

Problem is, at random times, my Mac would just shut down and then restart after I clicked through the "gray your Mac shut down without telling you because it's selfish" warning screen.

I took it to a local computer shop and they couldn't really tell, and suggested I do a clean install of Mountain Lion. I did that, got everything back on from Time Machine (just Documents and Music, no Libraries per the technician's instructions), and then last night it did it again. Restarted while I was working on a web site. No notice.

I wondered if someone could help me diagnose the problem or problems(s), since I can't make ANY sense of the sytem report.

Here is the complete system report when the panic occurred. Thanks for ANY help at all you can provide.

kevinloucat-

You kernel panics are all occuring as a result of the same routine, the GeForce driver.

But there are three different kernel panics in the four reports:

panic(cpu 2 caller 0xffffff800f2b7b95): Kernel trap at 0xffffff7f90a27975, type 13=general protection

panic(cpu 6 caller 0xffffff801c0b7b95): Kernel trap at 0xffffff7f9d7d0975, type 13=general protection

panic(cpu 2 caller 0xffffff800d2b7b95): Kernel trap at 0x0000000000000000, type 14=page fault

panic(cpu 2 caller 0xffffff8009051c07): "IOGMD: not wired for the IODMACommand"@/SourceCache/xnu/xnu-2050.18.24/iokit/Kernel/IOMemoryDescriptor.c pp:1424

Sep 29, 2012 12:23 PM in response to kevloucat

I'm also getting this panic (3rd time in 6 days) and I have 3 NVIDIA GeForce GT 120 cards with 3 LED Cinema Diplays on a 2009 MacPro

Sat Sep 29 10:31:45 2012

panic(cpu 6 caller 0xffffff8016851c07): "IOGMD: not wired for the IODMACommand"@/SourceCache/xnu/xnu-2050.18.24/iokit/Kernel/IOMemoryDescriptor.c pp:1424

I'm going to revert to the older IOGraphicsFamily like Betalogue (link below) since the driver upgrade coincides with my first crash on Sep 24 2012.

Solution (hack) for kernel panics in 10.8.2?

Oct 3, 2012 4:59 AM in response to kevloucat

Far be it from me to tell you what to do, but replacing a video card because of a software bug strikes me as a bit extreme, not to mention potentially ineffective. I think it's quite clear that these KPs are caused not by a defect in our GT 120 cards, but the very presence of more than one GT 120 cards in our machines. There is a bug in the updated graphics driver included in 10.8.2 that triggers this problem.

The fact that you don't get KPs when you remove one of your cards does not prove that the card is defective. In fact, I'd be willing to bet that if you removed the OTHER card, the KPs would cease as well. Now, you MIGHT avoid the KPs by replacing one of the GT 120 cards with a different model/brand of card, but there's no guarantee.

I still hope that Apple/NVIDIA is sufficiently aware of this issue and takes it sufficiently seriously that it'll be addressed soon in a software update soon.

Pierre Igot-

I wanted you to know that an update has been issued. It is not clear whether it addresses this issue:

About OS X Mountain Lion 10.8.2 Supplemental Update

The OS X v10.8.2 Supplemental Update is recommended for all users running OS X Mountain Lion v10.8.2 and includes the following fixes:

  • Resolves an issue that may cause certain Japanese characters to appear incorrectly in Mail
  • Allows Safari to access secure sites when parental controls are enabled
  • Addresses an issue that may prevent systems with more than 64 GB of RAM from starting up
  • Resolves an issue that may cause DVD Player to unexpectedly quit

.

Oct 29, 2012 4:22 PM in response to Pierre Igot

I crashed today. It's been 14 days since my last Panic attack. So as Pierre sez, "better than daily crashes"

I updated to 10.8.2 but it did not upgrade /System/Library/Extensions/IOGraphicsFamily.kext. I'm still on 2.3.4. Pierre, did you try 10.8.2 with 2.3.5?

I guess I'll try resetting the SMC.

PS: Not sure if this is related but with ML, my Mac always forgets my window arrangements. Whenever I reboot (or crash) I get a random layout. Anyone else getting this?
