Diagnosing cause of kernel panic on a Mac Pro

I've got an early 2009 Mac Pro with the following specs:

2.66 GHz Quad-Core Intel Xeon

16 GB 1066 MHz DDR3 ECC

NVIDIA GeForce GT 120 512 MB

OS X 10.8.1

Problem is, at random times, my Mac would just shut down and then restart after I clicked through the "gray your Mac shut down without telling you because it's selfish" warning screen.

I took it to a local computer shop and they couldn't really tell, and suggested I do a clean install of Mountain Lion. I did that, got everything back on from Time Machine (just Documents and Music, no Libraries per the technician's instructions), and then last night it did it again. Restarted while I was working on a web site. No notice.

I wondered if someone could help me diagnose the problem or problems(s), since I can't make ANY sense of the sytem report.

Here is the complete system report when the panic occurred. Thanks for ANY help at all you can provide.

Every time your Mac has a kernel panic, be sure to report the problem to Apple on the next start-up. Every report is important. A growing pile of reports help Apple's managers justify engineering time needed to fix the problem.

All your previous kernel panic log files can be found within Console.app:

Applications > Utilities > Console.app > System Diagnostic Reports

Originally I suspected this problem was a one off hardware fault. However, seeing multiple near identical panic logs suggests a software error.

Sep 18, 2012 9:35 AM in response to Graham Miln

Yeah, I found about all of these reports when I posted about this kernel panic earlier last week. I got some helpful responses, but nothing was solved. Most of the people got too stumped to help. They did mention these records. I disabled Logmein, which seemed to be the problem, and it was showing up in the list, but when I disabled it, AppleHWSensor seemed to be the culprit. I reinstalled that extension after hearing from some people that it was too important. I don't like monkeying with the brain of the Mac, that's for sure.

I DO send all the reports to Apple, so I've been on top of that.

Hopefully something will come out of this. I tried to look about upgrading my video cards, but couldn't find any options (larger memory) for my 2009 Mac Pro. Wouldn't mind getting more video memory if my machine could handle it.

Thanks again for the info!

Sep 19, 2012 1:39 AM in response to kevloucat

kevloucat, please keep us informed about what you find after removing the GeForce GT 120 cards individually. I will be interested to know if this problem continues with a single graphics card.

Be sure to check that any replacement graphics card comes with an Apple compatible firmware installed.

It is safe to rule out non-Apple extensions as a problem; I have no third party kernel extensions on the Mac Pro that suffers the panics.

I have begun to associate the panics to when I am using Safari (v6.0/8536.25). Panics have yet to occur while I am working with other applications.

Are you seeing any pattern in what applications are front most at the time?

Sep 19, 2012 8:21 AM in response to kevloucat

kevloucast-

All your Panics are occurring in the latest NVIdia GeForce display driver.

Retina MacBook Pro users are complaining of a kernel panics in the newest GeForce driver. Anecdote suggests there may be a memory leak in the driver. Apparently it is made MUCH worse by any but the latest version of Chrome.

I suggest you do the following as a temporary work-around:

1) if you use Chrome, get the latest version.

2) after long use, do a Restart. At least daily, possibly more often.

Sep 19, 2012 10:22 PM in response to Grant Bennet-Alder

The kernel panics continue with OS X 10.8.2 (12C54).

  • The problem is not unique to the Chrome browser.
  • The panics occur too soon after boot to be memory exhaustion - additionally, the kernel task is not growing beyond reasonably memory sizes when viewed by Activity Monitor.

The panic log now includes better information suggesting the root cause:

panic(cpu 10 caller 0xffffff8016651c07): "IOGMD: not wired for the IODMACommand"@/SourceCache/xnu/xnu-2050.18.24/iokit/Kernel/IOMemoryDescriptor.c pp:1424

This is an Apple bug within an Apple written/shipped kernel.

I have the exact same machine (2009 Mac Pro with 2 Ge Force GT 120 cards) and the exact same problem. It appears that I might have found a solution. It's a hack, but until Apple releases an official fix (which could take a while), it might help you eliminate the kernel panics without having to revert to 10.8.1:

Well, Betalogue was too good to be true. Followed the directions and the kernel panic happened again.

Going to remove that video card again and give that another shot.
