Boot to recovery by holding cmd+r and reinstall the operating system.

before reinstalling everything try boot to recovery disk, then running the Disk utility to repair permissions, and repair the disk. Also try the procedure to reset the PRAM:
power machine off, then turn back on and hold the key combination
<COMMAND> + <OPTION> + p + r
until you hear the start up chime three times
Then try resetting the SMC:

I did try to reset PRAM but it went keep on restarting / rebooting my mbp until i lift up my fingers

Yes, that is the expected behavior. It is recommended that you go thru at least two cycles (startup chimes) prior to release to ensure full reset.
