Battery not charging with new adapter

I have an early 2011 Macbook Pro with a brand new 85W power cable that won't charge most of the time. The light on the adapter will mostly be green, and sometimes even turn off. Shouldn't a new adapter, especially at the right wattage, be sufficient to charge at least most of the time?

MacBook Pro (17-inch Early 2011), OS X Mountain Lion (10.8.3)

I had a problem like this, then I fixed it. I was doing the SMC reset as on the page. Actually, just the preliminary things to try, and got it to work. In my case, unplugging the charger, BOTH ENDs, then plugging in the wall side first, then the device side, fixed the problem. (I keep this charger plugged in all the time so it never occurred to me...) If I unplug, then plug, the mac side again, the problem comes back.
