i too was finally issued three video credits. i used one to get episode 12. so, no problem.
at last, a reasonable response from apple support.

so i sent them something along the lines of:
".. . . ..the whole issue of the $2.99 is not a big deal. what is a big deal is the psychological impact on the customer.
consider this:
imagine that you were to go to a physical bricks and mortar store and preorder an item that will be out in a month. you prepay for the item and are given a receipt for it. you come back to the store in a month and find the item on the shelf with a price tag attached. you bring it to the counter and show the prepaid receipt to the cashier, who informs you that if you want the item now, you will have to pay for it again, and that what the store would prefer that you do is leave the item in the store, and wait for them to contact you at some indefinite time in the future.
you are left standing there with the item in one hand, the receipt in the other, and your mouth hanging open.
it is unusually galling.
the appropriate thing for the store to do would be to dispense the item to the customer immediately upon verification of the receipt.
in addition, if the customer has incurred the financial inconvenience of repurchasing the item, the store should cheerfully refund it. it is definitely not helpful to metaphorically "rap the customers' knuckles with a ruler" by refering them to the itunes "terms and conditions" that state "all sales are final". if the store was working properly, the customer would have neither the proclivity, nor even the _ability_ to pay twice."
hopefully apple has finally got it's head around this "mountain out of a mole hill" problem, because it's pretty much a p.r. no brainer.
