My Mactonish HD is now Untitled, I only have 4 options, download Mac OS X Lion, Safari for help, Time Machine and the Disk Utility.

Please - what happened? "Didn't work" can mean many different things.
Did Disk Utility / Disk Repair run to completion?
In Disk Utility, could you please check if the drive has any data on it. On the left, click on Untitled, and check the info on the bottom of this pane. How much is used.

OK, 156 MB means there is no OS on that drive. So let's go back to your very first post. You need to select download Mac OS X Lion. (Not Disk Utility).
It appears that you erased the hard drive, so there is no OS on it, which is why you get the question mark. So go through the OS installation. First download the OS, then instal it.
After you have done all that and the computer is booted in the new OS, just change the name of the drive, by clicking on Untitled (under the icon) and typing in whatever new name you want.
I'd like to get it back. When I boot the Mac it has this folder with a question mark on it so I was told to put it in Recovery Mode and now I don't know what to do.

From the list of options you have provided, it looks like you restarted the Mac in Recovery mode. Please provide some explanation of what you did and what you want to do.
The name can be changed any time, so first let's continue with Recovery.
Select Disk Utility and press Return. When DU runs, select your hard drive on the left hand column, and click on Repair Disk on the bottom right.
When that is completed, quit DU and restart your Mac.
Tried that many times, didn't work. :/
In finder under preferences go to sidebar and select show hard disks, then on the sidebar right click the drive and select get info and see if you can change the name there.
It's still Untitled, yes. Capacity: 60.32 GB Available: 60.17 GB Used: 156 MB Number of Files: 3
