I want to buy the new iPhone 5S in Germany. Is there a problem when I use the iPhone in Belgium? I heard there is always a worldwide limited warranty of 1 year. Is this correct?
There is NO world wide warranty on iPhones (or iPads)
However Apple treats the EU as one country for warranty purposes so a German iPhone is covered in Belgium

there is a country warranty which is recognised across the EU and only the EU

The reason why I want to buy the iPhone in Germany is because the 5S is in Belgium in December...
There is only warrantee in the country that you bought it in. Also in European countries (because the provider systems are not "homogenised").
What is interesting though that the EU made it obligatory that you have TWO years of warrantee. Apple is slowly complying to that. In BE you probably have the two year warrantee. I can imagine that you want to buy in DE because the VAT is seriously lower there than in BE, but you will not have warrantee in BE then.

I order the iPhone 5s online and let it deliver to an adress in Germany. Then, my friends bring the iPhone to Belgium so I can use him here.
If I don't have problems, there is no problem. But if I have a problem with the iPhone, can I go to an official Apple store in Belgium so they can send it to any place for repair?

You have to bring it to the country where you bought it directly, Apple can not ship it for you. Apple Stores abundant in DE, Oberhausen and Köln are the closest to BE.

Apple in Belgium will handle the iPhone in exactly the same way had you purchased the iPhone in Belgium
as long as it comes from another EU country .Apple do not repair they exchange for a refurbished unit if a warranty issue or charge for an Out of Warranty exchange unit if you have damaged or caused the issue through misuse
