You need to open the App Store and search for the game. It should show an iCloud logo rather than a purchase or Free button. Simply tap the icon and it will download again.
I can't download a deleted game that I clearly see in purchased. It shows up in the iCloud logo, but when I tap the logo, it just spins and spins but my game does not come back. When I go to iTunes on my computer, it's there, and it says downloaded, and won't let me down load. Help!!!
Or in the App Store just go to Updates->Purchased and find it under "Not on This iPhone"
