How can I reset my security questions if I have forgotten my rescue email address password and security answers?

You will need to contact iTunes Support / Apple to get the questions reset.

When they've been reset you can then use the steps half-way down the HT5312 page that you posted from to view and/or udpate your rescue email address for potential future use
