double check your do not disturb settings (see if there is a moon icon displayed beside your battery icon) if so, go to settings>Do not disturb and turn it off.
That is only setting where you can make the phone not to ring when it is locked,
it's under Settings>Do Not Disturb>"Silence"
"Only While iphone is locked"
if the settings are fine
Do a reset by holding the power and home button until the apple logo appears ignoring the slide to power off (no data loss).

Your post has made my life so much easier, you have NO idea - truly. THANK YOU
FOR SOME UNUSER-Friendly reason. The assistive touch status of muting can affect ringing when the phone is screen locked or not. Under accessibility options select ASSISTIVE TOUCH Then select the White circle Then select the Device icon of an Iphone Then select Unmute. (this will then show a button of a bell with a line through it)- Note this is the action the button will do when selected not the status of the process. Then exit the assistive touch/accessibility options

Did you ever get this resolved? I have the same issue: all swiches are off in Do Not Disturb, still no rings when locked.
This worked for me-copied post from another forum.

Check Settings > Notification Center > Phone. Make sure Alerts are "On." Make sure the alert sound is set to the ring sound you would like.
I double checked and the phone isnt on do not disturb. I tried the hard reset with holding the power and home button, tried calling again but still have the same problem??
