again my friend i have requested iphone unlocked and approved

almost 15 days waiting the phone is still unlocked

iPhone 4, iOS 7.0.4

Then the unlock is not working. That is what I have been trying to tell you. However, only AT&T can address this issue, you need to work with them. Include that request number in any correspondence you have with them as a reference.

Then call AppleCare and do as I said and make sure they received the unlock request.

Also, did anyone previously hack/gevey sim/jailbreak this iPhone to unlock it?

i did not put any gevey sim or anything else ...is this site or the admin only support discusion are they not supporting to help this issue...

No, you can call the AppleCare number like I suggested. Would be very simple to call and ask if it shows unlocked.

Have you tried erasing it like I said then connecting it and setting it up with the AT&T sim in it?

Yes what? You completely erased the iPhone, connected to your computer to activate it with the AT&T SIM in it? If you did and still got the sim not supported either it has not been unlocked by Apple (CALL THEM) or the phone was at some time jailbroken.
