Just to be clear, to enter a number of your passcode, you tap on the number, hear the number, then tap on the number twice more to enter it. It should register in the dots above, showing that it's been accepted. You don't use the Home button to do this.
Assuming VoiceOver is your Accessibility Shortcut, three clicks on the Home button should turn it off. They can be the same speed as you normally double click to access the app switcher.
Triple-click the home button, rather quickly, to turn it off. Voice over must have been enabled by a few bumps to your phone. It can be activated and deactivated when the phone is locked.

If the above answers don't work try resetting your Ipad by holding both the home and power buttons until the Apple logo appears. Then release both button. Wait until your iPad starts on it's own.
None of your suggestions work...so what now? Restarting does not help.
I am aware of the protocol but without success. Does the frequency of presses on the home button make a difference to the outcome?
