Use "Find My iPhone" an official app from Apple, this capp can be downloaded on another Apple device or can be used on http://icloud.com/

IS find my iphone on in your device? If it does go to icloud.com/find# choose you device and then on the map click you device and chose lost mode. Go back to your device then select erase.
