How to dissociate an iOS device from iTunes / iBooks purchase account

I want to pass my iPhone 4S to a neighbor's child after wiping my data from it. However, I have read in the tech press that this process does not dissociate one's iTunes / iBooks account from the device. I want to ensure that the device cannot be used to access my account purchases. How do I do that? I presume it has to be done before I wipe the device. I cannot find this info in the knowledge base documents (so far).

iPhone 4S, iOS 7.0.6

You can log out of an account by tapping on the id in Settings > iTunes & App Store

Also turn 'off' (assuming that it's 'on') Settings > iCloud > Find My iPhone, and log out of your account from iCloud : Settings > iCloud > Delete Account

If the phone is associated to your account then you might be able to remove it via your computer's iTunes : iTunes Store: Associating a device or computer to your Apple ID (or you can do it directly on the phone logging out of the account by tapping on your id in Settings > iTunes & App Store, tapping on 'View Apple id' on the popup and logging in, and there might, if the phone is associated to your account, be a 'remove this device' button). If the phone has been associated to your account for less than 90 days then you won't be able to remove it.

Also remove the phone from your support profile if it's registered to it : https://supportprofile.apple.com/

And, if you are on a contract, you've contacted your phone carrier to cancel it ?

Thank you so much for the help! (and so fast, even better)

The phone has long since become only a WiFi device so contract is not an issue.

Since you mentioned it though, I am wondering about the SIM card. The kid will need it in order to be able to use the old phone as a WiFi device but I don't want to leave it in if it links to me or my accounts at Apple or the carrier. Should I remove it and go buy another one for the kid or just leave it in there? I don't know anything about the data carried on a SIM card when the phone is not under contract and no longer activated for use as a phone.
