Payment cards are removed from Apple Pay on iPhone and iPad if you do either or both of the following:

I had problems syncing my Calendars, so i logged out of my iCloud account in order to rte log-in and after i've done that i received a notification from AMEX, that my card was removed successfully, so i checked the Passbook, didn't have any of my cards (there were three). After calling Apple and being on hold for good 25 minutes, i finally was advised to call each of my banks, since Apple doesn't authorize removal of cards. So i did call one of my banks, and they said they do not know what might happen - what i mean it was a classical circuit of "you have to call there"... It ended up that ti just added the cards again. Anyway, we'll see. I guess the system is pretty new and some glitches are expected.
This is happening to me constantly ... every week or so. I am not going to keep reinstalling my cards ... sort of defeats the purpose since I can't actually carry "just my iPhone because I never know if the cards will or won't be there when I'm standing at a checkout station ...
There should be a notification to tell you WHY it is happening so you can modify your login/logout behaviors or otherwise mitigate the triggering event(s).
This happened to me as well but the cards that were supposedly removed have not been in my apple pay for months aside from one of them.
And the one that I still use with apple pay is still showing in my apple pay.
These almost seems like a tempted hack to obtain information from people.
Did you subsequently turn off Passcode? I have seen that when you enable Passcode, enter a card in Passbook or settings, then later turn Passcode off, the card goes away, even if you turn Passcode back on. Very irritating.
I saw the same thing when I logged out of iCloud. I’m sure this is due to security concerns. I am not the least bit irked or annoyed. I am grateful to Apple for taking security seriously. This is no glitch. This is security at its finest. If convenience is more important than security then don’t use Pay.
There are already plenty of other forms of security in place that makes deleting my cards when I turn off passcode unnecessary. The whole point of Apple Pay was to be convenient, but making me enter a passcode every time I access my phone dozens of time a day is just the opposite.
I agree with bsb88. The only security to be gained is if you lose your phone or have it stolen, and don't remotely lock it. I really doubt I would have to worry about that. The right way to do this is to "hide" the cards when Passcode is disabled, then show them when re-enabled.

I would like to be able to use Apple Pay, but I don't want to have to use a passcode (or use Touch ID) for accessing my phone. There are times when I occasionally use Touch ID, like when I travel and the risk of losing my phone in a public place goes up. Otherwise I keep it off for convenience. I set up a few bank cards, and when I turned off the passcode, all of the cards were deleted from Apple Pay.
Having same issues w multiple cards being removed and stating ininitiated.
I did not initiate it.
Chase and other cards say it's not initiated by them and that I'm not Enrolled in Apple Pay.
Apple what is going on??
I have not done neither of those two things and yet all my credit cards were removed. I spent an hour on the phone last night between two of the companies exploring the reason, in case it was fraudulent. Not a good experience from apple. To my knowledge there was not a communication from apple stating why this happened either.
Well then send feedback to Apple requesting a change. If enough people complain things usually get attention.

Wondering if adding the same card to 2 phones causes this? This is what seemed to happen to us but we need to do more testing.
Just talked to the credit card company an they said i logged out of iCloud, I never logged out of iCloud so something is wrong here.
