Mobile Safari Crashes on Credit Card Autofill prompt iOS 8.1 iPhone 6

We are experiencing a consistent problem with mobile Safari crashing when prompted to remember a credit card on our checkout page. Regardless of what option the user chooses (Remember this Card, Never for this Card, or Not Now), Safari immediately crashes when one of these options is chosen. This happens on an iPhone 6 running iOS 8.1. This happens every time on that particular iPhone 6, but doesn't happen when we test on other iPhone 6 Plus phones running iOS 8.1. We have also had this problem on the iPhone 4S with iOS 8.1 though on that phone it only happens sometimes.

Other sites that prompt for the credit card autofill do not cause the crash, so it seems there is something specific in the HTML/CSS/Javascript on this page that is causing the problem? Tapping any of the 3 options causes Safari to crash.
