My daughter's new iPhone has my ID on it. Our apple accounts seem to be mixed up - she has used my iTunes account with her iPod in the past. How do I disassociate our accounts & get her ID on her phone?

Also - since you have indicated iOS 8.x - you can go into Settings --> iCloud --> scroll all the way to the end --> Sign out -
