Your Preview is putting both a checkmark and an X into the checkbox. My Preview only puts a checkmark. I can't see where you can specify either one or the other (X or checkmark) in Preview on my Yosemite 10.10.1 with Preview version 8.0. Did you do something to enable "X" rather than checkmark?

What version of OS X and Preview are you using?

I use an app called PDFpenPro to do this sort of thing with usually (fill out forms and editing PDF files) and it also only puts checkmarks into the checkboxes.

I don't know if you ever solved this but for future reference.

It seem Preview in OS X has a capability of making a normal PDF (non-editable) editable and I'm guessing that it's doubling up on top of the editable symbol that's already in your editable PDF. I had the same problem and I was making my PDF editable with Adobe Acrobat Pro. I fixed the issue by changing the appearance setting in properties for the checkbox.
