I have a pre-paid iPhone 4s with verizon (no SIM card). I called Verizon and I got an "unlocking code" but no instructions on how to do it. What is the procedure to unlocking the phone so I can use it with other carriers? I plan to use it with GMS carriers within USA and abroad.

I've not heard of the carrier providing you with a code, since there is no place to enter/upload any code. What did Verizon tell you about entering this code?

Regardless of the change, it still doesn't apply as iPhones are not sold as "Phone-in-a-Box". Those are phones sold in a plastic bubble pack. The rep that the poster spoke to may have been confused.
Good news. Sadly, that rep isn't going to have a job very long if they keep forwarding internal memos outside the company.
